you can use a shaft collar. I figured out how to do it with the shaft collar after I had already made the controls so never did make it this way. Hard for me to explain how though. I looked at pics of forward controls and figured it out that way....

You may have better luck finding this part...

You are not authorised to download this attachment. That is what it tells me when i click on the zip file. Also i don't see any pics that are posted, other then one pic on the last page.

I just downloaded the Template Update zip file and I can see it but it will not let me print it. Can someone help me find a copy I can print?

Also the PDF file link says the file does not exist anymore.

Problem is with the way you try to print it, however that may be, no one else had issues. If you tell me what format you'd like it in besides PDF, I can help, but I prefer to keep in PDF format.

Where did you get the shift collars? Is that the prototype you you mentioned?

That is not mine, I do have some shaft collars I purchased locally at Tractor Supply. They are also found on Ebay .
